The Nurse Practitioner provides comprehensive primary health care and health education to a defined population of adult patients in the clinic or in the patient’s home. Provides clinical services as advanced practice registered nurse.


The Nurse practitioner will work collaboratively with others to provide health promotion, health education and sick-care services in an effort to improve our patient’s quality of life and to facilitate optimal health outcomes through cost-effective healthcare. The nurse practitioner may provide nursing and medical services to our patients, diagnose, order DME and order/request referrals. The nurse practitioner also will perform annual comprehensive wellness assessments and emphasize the importance of disease prevention by incorporating health goal review, teaching and counseling with members and their families. The nurse practitioner will report to the Clinic Manager and work closely with Peoples Health Market teams to ensure priorities and objectives are accomplished.


Principal Responsibilities:


  • Performs and documents physical examination and accurately records history, medications, allergies, and review of systems on adult patients.
  • Provides primary care, including ordering and evaluating diagnostic/screening/laboratory tests, and management of minor acute illnesses.
  • Conducts comprehensive wellness assessments on members either in the clinic or patients home.
  • Assess, diagnose and manage chronic conditions for patient population.
  • Identifies deviations from normal, initiates and evaluates treatment for patients with commonly occurring health problems within their scope of practice.
  • Consults, co-manages or refers as appropriate.
  • Prescribes medications as delegated in the protocols and/or procedures within licensed prescriptive authority.
  • Provides individualized patient teaching and counseling.
  • Completes necessary documentation in a timely manner.
  • Develops, implements, and/or participates in research or educational activities.
  • Maintains competency in clinical practice and meets and maintains requirements for certification.
  • Participates in quality improvement activities and monitoring of health outcomes for patients.
  • Ensures coding and documentation of HEDIS Quality star measures and Risk Adjustment diagnoses as appropriate.
  • Ensures proper documentation of diagnoses and appropriate coding.
  • Facilitates clinical communication with referring PCP regarding member status as appropriate.
  • Provide member clinical services as referred by member’s PCP.



Physical Demands/Working Conditions:

  • The work involves considerable physical exertion such as frequent lifting of patients and equipment, bending and stooping, and walking. The work environment involves moderate risks or discomforts, which require special safety precautions, e.g. working with risk of exposure to contagious disease or infection, or working with emotionally disturbed patients. Precautions are routine for nearly all situations. The nurse practitioner may be required to use protective clothing or gear to continuously maintain universal precautions.
  • Must be able to travel between corporate office, clinical offices and member's locations as needed.

  • Good time management skills to effectively see members and document visit in clinical note within structured timeframes.
  • Excellent clinical, communication and interpersonal skills required for effective day to day patient management to assure a high quality of patient care rendered.

  • Ability to work in an environment requiring innovation, self-motivation and adaptable to change


  • Successful completion of an advanced practice Nurse Education Program
  • Experience in a managed care and/or ambulatory environment preferred.


  • Current, active, and unrestricted license in the State of Louisiana as Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN)
  • Certification as an Advanced Practice Registered Nurse in the areas of Family Nurse Practitioner or Adult Nurse Practitioner by National Certifying Agency
  • Current, active, and unrestricted prescriptive authority in the State of Louisiana and in accordance with the designated Collaborative Practice Agreement or actively apply for and receive a PA within 120 days of date of hire.
  • Maintains current BCLS certification.
  • Valid driver’s license issued by the Department of Motor Vehicles required.
